Lancet Diabetes & Endocrinology Type 1 diabetes Commission

Objectives

To explore and document best practices, gaps and challenges to improve quality of life and care for people with type 1 diabetes (T1D).

Approach

Despite a century of progress in managing type 1 diabetes (T1D), there has never been a comprehensive, person-centred global analysis that addresses the societal and health system needs of people living with T1D across different life stages, geographies, and income levels.

Therefore within this project, the Lancet Diabetes & Endocrinology Commission on type 1 diabetes is set out to deliver a comprehensive synthesis of existing knowledge and research, complemented by in-depth country case studies. These cases will reflect geographic diversity and pay special attention to gender, occupation, and the voices of people with lived experience. The Commission’s work is especially timely, aligning with the World Health Organization’s global target for T1D care and the World Diabetes Foundation’s growing commitment to this area.

The Commission aims to identify and prioritise the challenges that hinder optimal T1D management, education, and empowerment. It will propose actionable recommendations to improve the lives of people with T1D—particularly in low- and middle-income countries (LMICs)—and to guide innovation in care delivery and health system responses.

The Commission’s findings will culminate in a landmark report to be published in The Lancet Diabetes & Endocrinology.

This report will:
• Document the current global landscape of T1D care.
• Present new data to address knowledge gaps.
• Highlight systemic and societal barriers to effective care.
• Promote evidence-based strategies for equitable and sustainable improvements in T1D outcomes.

By bringing together global expertise and lived experience, the T1D Commission will help shape a more inclusive and effective future for T1D care worldwide.

Expected results

- Review of education models and their sustainability; on psychosocial and gender impact of T1D; and adult care carried out and published.
- A report based on mapping and documenting the experience of WDF partners in education and training programmes along with research on quality of care in T1D published based on SWEET registry data on T1D from high-, middle- and low-income countries.
- Four expert consultations carried out and published on 1) recommendations for health system set-up for the management of T1D (consultation, levels of care); 2) for registry, data sets and indicators needed for T1D; 3) life changes and transitions in T1D; and 4) mapping of legislation needed to ensure “positive” T1D environments.
- Articles published on 1) investment case for process of prioritisation for governments published; 2) geographical impact on optimal diabetes care and insulin delivery; 3) model of interventions to prevent premature mortality; 4) study on Universal Health Coverage and diabetes.
