To strengthen diabetes care at primary level and within communities through a patient-centred approach, targeting three states of Mexico.
1. Through project WDF15-1235 (pilot project, expected completion Q2-2018) MCI has mobilised health system support in Mexico, also at federal level, towards the ‘microclinic’ approach.The pilot stage in Puebla state is now outlined to be scaled up in Puebla and replicated in two neighbouring states through the following activities:2. Establishing a coordination group consisting of MoH federal and state levels and MCI, under the framework of the Mexican national strategy of prevention and control of diabetes; Institutionalising the patient support model piloted under WDF15-1235 into existing MoH structures and programmes incl. ‘self-help groups’ (‘GAM’/’PREVEGAM’), local community health/environment and mobile health services.3. Training of HCPs and CHWs across the three states based on material from WDF15-1235 and as per the newly agreed upon integration with existing programmes; All trainings to be considered integral to CME and CHW sensitisation already owned by the MoH.4. Capacity building of primary health care units across the three states targeted and ‘care networks’ established between units, CHWs, and the patient-support groups with emphasis on family and household level mobilisation, counselling and support; Data collection to be included.5. Roll out of further awareness and advocacy activities targeting policy level as well as broader population segments.
