Cambodia project for early detection and management of diabetes in primary care

Objectives

The goal of this project is to pilot a primary care and community level approach for early detection and management of diabetes and hypertension.

Approach

This project intends to demonstrate the feasibility of implementing a holistic, primary care approach to early detection and management of diabetes and hypertension at the provincial level in Cambodia. The project design is drawn from models that have been shown to be effective in the Cambodian context, and incorporates training and support for implementation of a newly adopted Ministry of Health Standard Operating Procedure for Diabetes and Hypertension Management in Primary Care. It will also include the development of community-based support for diabetes patient information, education and care through partnerships with well-established groups and organisations that provide support at community level.

The project will promote a primary care strategy in which health centres play a new and vital role in screening, referral, and community-based support for their patients, with direct linkage to newly established district clinics with staff trained under this project. The third objective is to establish an effective referral system and information systems that improve follow-up care for patients diagnosed with diabetes. The size and scope of the project will aid in evaluating new and innovative models for providing community-based diabetes patient education, information and care that can be both effective and financially sustainable.

Results at completion

Six secondary hospitals strengthened and six diabetes clinics established at primary health care centres
8 doctors and 8 nurses at secondary level, and 87 HCPs trained in prevention and management of diabetes and hypertension
493 community-based health volunteers trained
12 SC meetings and five workshops held for provincial representatives
52.5% of adults of 40+ years in target areas screened for diabetes and hypertension
5,695 people diagnosed with diabetes and hypertension, treated and trained in self-management
Several M&E process and clinical indicators (incl. WDF indicators) monitored, and aligned within government HMIS
