Families united against diabetes, Nicaragua

Objectives

To strengthen capacity within diabetes prevention and care through primary health and community level interventions

Approach

Establish a project steering group consisting of INPRHU, health authorities and other key stakeholders; steering group to develop training and awareness material incl through active engagement with current WDF project partners in Nicaragua (a leading university and two civil society agencies; WDF15-961) in order to adapt material from these. Also, the PAHO-produced training manual for CHWs (WDF14-914/WDF10-562 in Mexico) will be adapted.

Capacity building of health care system across municipalities targeted through training of primary level HCPs and CHWs, and through provision of basic equipment.

Sensitisation and mobilisation of targeted municipalities through a range of community level and awareness activities, followed by roll-out of screening and education campaigns, integrated with the primary level clinics involved.

Implementation of a ‘community based rehabilitation’ model aligned with existing MoH primary care/community integrated approach, and also aligned with the PAHO/CARMEN initiative, including a series of activities and interventions i.e. self-help groups, diabetes patient clubs, media campaigns, psycho-social support, ‘learning fairs’, etc.

Close on-site community and patient/household level monitoring and evaluation, incl on basis of baseline/endline survey and data collection (with WDF aligned indicators).

Results at completion

42 primary health care centres/posts in four municipalities strengthened
40 HCPs (nurses) trained (70% female and 30% men)
160 CHWs trained (60% female and 40% men)
2,420 patients treated and provided care at strengthened clinics
2,420 within target for glucose control
5,800 people at risk reached through awareness activities, 70,000 people reached with awareness messages through radios etc.
18 diabetes clubs in four municipalities were established or strengthened
