Community health prevention and management on diabetes and high risk group, China

Objectives

To implement a pilot phase of a provincial diabetes care and prevention programme in Jiangsu Province, China.
Through health intervention, life style management, training and other cost saving intervention methods, the aim is to change high-risk behaviours and improve knowledge and awareness of diabetes and thereby improve the quality of life of people with diabetes. Their self-management ability will be improved as well, especially the compliance of patients in rural areas.

Another purpose of the project is to enhance the capacity of community doctors and health education professional teams through training. As the community doctor is the gatekeeper for finding and managing patients, his/her ability to manage and control diabetes is significant.

Approach

Project activities include:

Roll out of a baseline survey on prevalence and knowledge of diabetes in six municipalities in Jiangsu Province.

Development of information material on prevention, treatment of diabetes and education of HCPs and patients, including training in behavioural change and diabetes self-management.

Capacity building of community health centres through basic diabetes training of health centre directors, HCPs, community doctors and nurses.

Development of a web platform for interface between patients and doctors used by doctors to follow patients’ blood glucose levels and provide clinical guidance, called the ‘mHealth’ component.

Focus on the importance of diabetes prevention and control enhanced through awareness raising activities, including radio broadcasting, poster campaigns and other media activities.

Results at completion

Baseline-, midline and endline surveys in Jiangsu Province conducted, covering 7,000+ people.
78 community health centres strengthened and 78 directors provided basic 2-day training on diabetes, including prevention and treatment
1,326 health educators provided basic 2-day trainings on diabetes care
706 community doctors and 508 nurses provided 2-day training on basic diabetes prevention, diagnostic and management
70,000 people with diabetes educated on diabetes self-management and monitored
