To develop an integrated care system to improve the quality of Type 1 diabetes (T1DM) management in the state of Gujarat

Objectives

  • Establish multiple T1DM clinics in the state of Gujarat
  • Educate physicians, pediatricians, family physicians, physical health care (PHC) doctors, clinical health care (CHC) doctors, medical college doctors, nurses, social workers, diabetes educators, as well as people with type 1 diabetes, and their family members on T1DM
  • Spread awareness regarding T1DM, its complications, and treatment by using appropriate tools
  • Create data registry of T1DM patients


Approach

Diabetes Care India will establish an enhanced framework for managing T1DM throughout the state of Gujarat. Project activities include:

  • Establishment of T1DM clinics throughout Gujarat through provision of equipment and supplies for T1DM management
  • Training of HCPs in comprehensive T1DM management
  • Education of T1DM patients, family members and school teachers through group and individual education sessions
  • Roll out of awareness activities regarding T1DM through awareness camps
  • Establishment of a T1DM patient registry in Gujarat (incl. the use of mobile health application)


Expected results

  • 20 T1DM clinics established in 15 different cities in Gujarat
  • 40 medical doctors trained
  • 2,000 nurses, paramedical staff and diabetes educators trained
  • 1,000 patients and their family members educated
  • 1,000 T1DM patients registered with clinical outcomes
  • 150 awareness campaigns conducted
  • 100 education programmes conducted
  • Registry of several clinical indicators (also HbA1c)
