Building on a successful pilot intervention in two rural municipalities to upscale screening, education and diabetes care services to rural areas in Antioquia
Objectives
To improve diabetes care and prevention in rural areas of Antioquia Department, Colombia
 
Approach
Preparatory phase including MoH / CLID selection of intervention sites
Strengthening of public health facilities and provision of basic equipment and supplies, incl. portable retinal camera for outreach eye screening
Capacity building of HCPs from targeted health facilities within diabetes care practices (incl. diabetic foot), with training provided by CLID
Risk assessment (FINDRISC) and screenings of general population organised at targeted health facilities
Screening for complications of diagnosed patients with a focus on retinopathy (a retinologist will be available for evaluation of photos via web) and diabetic foot, with referrals to more advanced services in Medellin (capital of Antioquia)
Roll out of education programme for patients diagnosed with diabetes in self-care through development of educational material, educational groups and individual sessions at health facilities
Roll out of awareness programme for the general at-risk population 
Introduction of an electronic patient registration system with various clinical indicators
 
Expected results
20 public health facilities in 20 municipalities strengthened
100 public HCPs (50 doctors and 50 nurses) trained in diabetes management
4,000 people assessed for risk, screened and educated in healthy living
1,000 people diagnosed with diabetes (based on high risk screening) and screened for complications
1,000 patients registered in EMR
1,600 people (1,000 with diabetes and 600 at high risk) educated in self-care and prevention. Total population of approx. 250,000 who would potentially benefit from the improved diabetes care capacity
